What documents do I need in order to apply for a passport?

If you are an adult applying for a 10-year passport for yourself, bring your current passport and one biometric photo when you come to your Consulate appointment. Additional documents may be needed if your data has changed—for example, if your last name changed because you got married, you will need a copy of your Polish marriage certificate.

See detailed information on how to apply for a 10-year passport >>

If your last Polish passport expired before  January 1, 2002 or you have never held a Polish passport, you will probably have to obtain a confirmation of Polish citizenship.

If the passport is a first-time passport for a minor (under 18), the application must be submitted by the parents of the minor, who must also present valid identification documents. At least one parent must have a valid Polish passport or ID card. A copy of his or her Polish birth certificate must also be submitted.

See detailed information on how to apply for a 10-year passport for a minor >>

If you qualify for a passport fee discount, you may need additional documents, such as a retiree/pensioner card or school/student ID. The Consul may request additional documents in the event of questionable data or discrepancies in the personal data.

Do I need to apply for a passport in person?

Yes, you must come to the Consulate in person to apply for a passport. On the day of your appointment, we will determine your identity (based on the Polish passport or ID card). We will also collect your biometric data (facial image and fingerprints) to be stored in the passport in electronic form. An application for a passport for a minor (under 18) is made by the minor’s parents. A child who is already 5 must attend the interview.

You cannot send your passport application by mail or through any other person, including family members.

Where can I apply for a passport?

At any passport authority office. In Poland passports are issued by the Voivode, and abroad by the Consul. You can apply for a passport at any Polish Consulate General or Polish Embassy in the United States and Canada. However, you should remember that you can collect your passport only at the same office where you applied for it. It is not possible to send a passport between countries, for example, from the Voivode's Office in Poland to the Polish Consulate in New York. Also, you must remember that matters other than those related to passport issues, such as vital records registration, can be handled only at the Consulate that corresponds to your permanent place of residence.

Before getting a passport, a Polish citizen must also obtain a Personal ID Number (PESEL). To get your Personal ID Number, you must present a copy of your Polish birth certificate.

How many months before my current passport expires can I apply for a new passport?

However, if passport replacement is not caused by a change of data or lack of space for visas in your passport, you must pay the full fee for issuing the new document. You should apply for a new passport approximately two months before your current passport expires.
