  • CHANGING PASSPORT DETAILS

    Persons applying for a passport bearing a different name due to marriage abroad must submit a copy of the Polish marriage certificate (see: registration of foreign vital records documents in Poland).

     

    Persons applying for a passport bearing a different name (first name, name(s), last name) that was changed as a result of statutory/administrative changes, must submit a new birth or marriage certificate (see: administrative changes of first/last name).

     

    Persons who entered into a marriage in Poland or abroad that resulted in the change of personal details (last name), must apply for a new passport immediately, but no later than within 90 days of the receipt of a final administrative decision or a final court decision confirming the change or processing of the marriage certificate confirming the change of name, and in the case of a marriage concluded before a Consul or a foreign authority, from the date of service of a copy of the marriage certificate.
